Long Term Care
Welcome to Long-Term Care at Hornepayne Community Hospital
At Hornepayne Community Hospital’s Long-Term Care (LTC) facility, we are proud to provide a warm, home-like environment where quality care and personal attention are our priorities. We share the hospital’s mission and vision and are located right in the geographic center of Ontario. Our 12-bed licensed facility serves residents primarily from our local community.
Our Facility

Our LTC unit offers both single and double rooms that residents are welcome to personalize. The space includes:
-
A bright and spacious dining and lounge area where meals and daily activities take place.
-
A large living room with a 65” television, DVD/VCR, stereo system, and a Wii console for family enjoyment.
-
Two large outdoor gazebos and 10,000 square feet of fenced yard for safe outdoor recreation.
-
On-site hairdressing and laundry facilities.
-
Spacious tub and shower rooms with mechanical lifts for safety and comfort.
-
Air conditioning installed in all 10 resident rooms (as of May 2023).
Our Team

We are staffed 24/7 by a dedicated team of Personal Support Workers (PSWs), Registered Practical Nurses (RPNs), Registered Nurses (RNs), and our Medical Director. We are also fortunate to have:
-
Our own Behavioural Support Ontario (BSO) nurse, who supports residents, assists with activities and infection control, and also happens to be a licensed hairdresser and avid gardener.
-
A RAI Coordinator who also provides hands-on care.
-
An Activities department that promotes social connection, organizes pet visits, and brings in outside entertainment, including local schools and EarlyON programs.
-
A Physiotherapy Assistant who works under the guidance of a contracted Registered Physiotherapist for regular resident care.
Our Mission

We strive to continuously improve our LTC setting while maintaining a safe, supportive, and person-centered care environment that feels like home—even within a hospital setting. We follow all Ministry of Long-Term Care guidelines and mandates to ensure excellence in service delivery.
Continuous Quality Improvement

Our goals include:
-
Enhancing residents' daily living and overall well-being.
-
Promoting safety, comfort, and meaningful activities.
-
Measuring progress formally and informally to guide improvements.
Our physiotherapy services have proven effective in maintaining resident mobility and stability.
Projects & Improvements

We are actively working on improvements, including:
-
Outdoor Garden Patio: A new garden patio is in the design phase for resident and family enjoyment.
-
Security Upgrades: New secure doors have been installed to keep residents safe while preventing unsupervised wandering into hospital areas.
-
Future Renovations: Plans are being made to remodel the kitchen/dining area and refresh our two indoor-style gazebos, with input from the Resident and Family Council.
